MAY 17

Krispy Kreme Doughnuts, 4906 West Broad Street – The following violations were reported during a routine inspection: uncovered employee beverage incorrectly stored next to good on table in drive-thru area; back hand sink and dish room hand sink don't have hand soap or paper towels; observed three bins of unidentified dry goods outside of their original containers; an employee with long flowing hair in a ponytail isn't wearing a hair net; drink stirrers incorrectly stored in cups of standing water; dish room hand sink is in disrepair; mold found inside carts, outside of equipment and on wall under hand sink; there is food debris in floor drains and grates; cables above equipment and ceiling panels/vents in front counter area and dry storage area are dusty; tiles throughout kitchen are cracked, particularly in back donut production area; back floor is not sealed.

Rapid Pizza, 1592 Mountain Road – The following violations were reported during a routine inspection: back hand sink doesn't have hand soap; observed buildup greater than one day old on slicer; temperature of tomato sauce in hot holding unit is too low; no date marks found on any prepared items; garlic and oil, pizza and fries are left out on time (time needs to be marked); car oil incorrectly stored next to sauce on back shelf; observed dirt and grime on racks and equipment along cook line.

Dugout Bar & Grille, 1510 Mountain Road – The following violations were reported during a routine inspection: no handwash signage is posted at kitchen or bar hand sinks; hand sink was blocked by coffee maker when inspector arrived; temperatures of lettuce, cheese and sour cream in standing one-door reach-in are elevated; air temperature of this unit is elevated (fans are blocked causing a lack of air flow because there are too many products in the unit); silverware and other utensils are not stored inverted; facility couldn't provide test strips for three-compartment sink; hood filters are dusty and greasy.

Subway inside Fas Mart, 2650 New Market Road – The following violations were reported during a routine inspection: no certified food protection manager is on site; facility doesn't have a verifiable employee health policy; facility doesn't have written procedures for cleaning up vomit/diarrhea; multi-quat sanitizer level at three-compartment sink is too high; lowboy unit doesn't have an ambient air thermometer; lowboy unit is not maintaining foods at the proper temperature; sanitizer isn't dispensing properly at three-compartment sink; ceiling/vent in back area is dusty.

The Daily Kitchen & Bar, 12201 West Broad Street – The following violations were reported during a routine inspection: hand sink at bar area doesn't have paper towels and it is obstructed by spray bottles and sanitizer bucket; raw chicken incorrectly stored above raw beef items in walk-in cooler; container of raw beef incorrectly stored directly on top of cooked pasta; mold found inside ice machine; can opener blade has food debris on it; there is buildup more than 24 hours old on soda gun nozzles; food debris found inside salad spinner; temperature of vegetable stock made yesterday is elevated; temperatures of spinach dip and au jus gravy in steam table are too low; unsalted butter left sitting out at room temperature; sauce bottles on the expo line are on time control but time is not being tracked; observed several cans of pesticide throughout kitchen (cannot be used); cut lettuce and raw hamburger patties were not cooled to the proper temperature before being placed on the line; observed small flies throughout the kitchen and bar area; wiping cloths stored in sanitizer but the quat concentration is too low; to-go boxes above expo station are not stored inverted; knife on the cook line has a duct-taped handle; plastic single-service food containers are being used multiple times; nearly all metal racks in the facility are either heavily rusted or beginning to rust; cream lowboy cooler has a torn gasket; several areas are dirty (drip trays, table legs throughout kitchen, dry storage shelves, top shelf of expo, refrigerator gaskets, dish racks, walk-in cooler fan guards, caulk in dish area, portable kitchen fans, cook line monitors, exteriors of bulk ingredient bins, refrigerator and ice machine vents, and inside cream lowboy unit); observed encrusted grease buildup on surface and along edges of panini press; underneath and behind cook line wall is greasy; wall behind dishwasher has mildew; floors underneath equipment have soils and food spills; ceiling vents throughout kitchen are dusty; many surfaces in beer cooler are moldy; light in dish area is missing protective shield.

Dunkin Donuts, 10600 Patterson Avenue – The following violations were reported during a routine inspection: written procedures for cleaning up vomit/diarrhea couldn't be located; quat sanitizer test strips are unusable because they’ve gotten wet; mop sink doesn't have a dual check valve installed.

Wendy's, 4609 Williamsburg Road – The following violations were reported during a routine inspection: sliding side door of dumpster was left open; observed grease buildup under burger flat top; there is low grouting on floor throughout kitchen.

Hawk's Fish & BBQ, 3050 Williamsburg Road – The following violations were reported during a routine inspection: facility doesn't have written procedures for cleaning up vomit/diarrhea; cook hasn't washed his hands since arriving to work; hand sink doesn't have paper towels or hand soap; ice machine has a layer of black mold inside; ice scoop is dirty and the handle was stored touching the ice; microwave is very dirty with food debris; temperature of pork butt cooked last night is elevated because of improper cooling methods; temperatures of potato salad, coleslaw, raw chicken wings, raw shrimp and milk are elevated; pulled pork and pork butt cooked yesterday are not date marked; sugar in a large bin is unlabeled; observed a full fly sticky trap hanging from the ceiling and full sticky traps for German cockroaches in the corner of the ceiling (remove when full and replace with new traps); one live German cockroach found in the front service area; observed many flies throughout the facility; dirty water is pooling under reach-in cooler #6; there are heavy grease deposits throughout kitchen which is creating ideal harboring conditions for pests; service area lowboy unit has a torn gasket; soap dispenser at hand sink is broken; racks in reach-in cooler are rusty; dry storage racks are dusty; homestyle refrigerator is dirty with food buildup inside and outside; rolling rack shelf on the cook line is covered in grease and food debris; fish juice is covering the bottom of reach-in cooler #5; mop is not being hung to air dry; non-working prep unit and glass door unit in side room should be removed from the facility.

MAY 19

Papa John's Pizza, 8903 Patterson Avenue – The following violations were reported during a routine inspection: three dented cans found on rack; temperature of grilled chicken in prep top unit is elevated because it stored above fill line; there are incorrect prep and discard dates for several items in sandwich unit and walk-in cooler (three cheese, philly steak, meatballs, Canadian bacon, sausage, beef, chicken and string cheese); temperature of tomatoes is elevated because of improper cooling methods; single-service boats are not stored inverted; walk-in cooler has a torn gasket and rusty racks; observed debris on racks in walk-in cooler, shelves throughout facility, outside of light bulb covers above convection oven and in between gaskets throughout; toilet in employee restroom is dirty on the outside and has buildup inside the bowl; there is low grouting on floors throughout; ceiling tiles around vent in front of house are dusty; observed debris/buildup on walls and floor at three-compartment sink.

MAY 22

Zaxby's, 11350 Nuckols Road – The following violations were reported during a routine inspection: hand sink has hand sanitizer instead of hand soap in dispenser; two fryer baskets are in disrepair; observed dust and food debris on monitors, cables, storage shelves, inside reach-in coolers and on fan system in walk-in cooler; there is a buildup of trash and food on floor behind cook line; ceiling panels and vents are dusty.

Sbarro inside Fas Mart, 11390 Nuckols Road – The following violations were reported during a routine inspection: temperatures of chicken wings in warmer unit are too low; the hot water food warmer where wings are stored is cutting on and off and not maintaining food at the proper temperature; observed several small flies in mop sink area and under three-compartment sinks; observed heavy ice buildup around walk-in freezer door; there is a leak at the mop sink chemical dispenser; floor tiles near mop sink are broken.

KFC, 4821 Williamsburg Road – The following violations were reported during a routine inspection: multi-quat sanitizer level is measuring at 0 ppm at three-compartment sink; plastic bins are stacked while still wet; there are food crumbs in the bottom of the reach-in freezer; racks in back corner of kitchen are greasy; hand sinks are slow to drain; mop sink doesn't have a dual check valve installed; floor under fryers is greasy; walls and floors of the chicken walk-in cooler are dirty and have an odd smell.

MAY 23

Eat 33, 6901 Staples Mill Road – The following violations were reported during a routine inspection: a food prep employee wiped his gloved hands on his soiled clothes several times and didn't change gloves and wash hands; mashed potatoes are in a deep container and cut tomatoes are covered during the cooling process; fryer baskets are in disrepair; observed food debris buildup on wire storage racks in walk-in and dish area and inside reach-in freezer.

Kabab House, 3411 Old Parham Road – The following violations were reported during a routine inspection: can opener is soiled; temperature of goat found sitting out on the counter is elevated; date marks are missing on several containers of cooked foods, rice pudding, tandoori chicken and chickpeas; flour bin is unlabeled; roach feces found around outlets and on top of water heater; dead roaches seen on the floor; glue boards for roaches are full and need replacing; blender, grinder, pots and single-service items incorrectly stored directly on the floor; walk-in refrigerator is not operating; light above three-compartment sink isn't working; several areas are dirty (three-compartment sink, interior of upright freezer, inside employee reach-in, microwave interior, flour bin, area between tandoori oven and grill, walk-in shelves and chest freezer gasket and ledge; chest freezer needs defrosting; caulking around three-compartment sink and floor edges and walls in walk-in are soiled; floor drain of prep sink is dirty; there is soot on walls and ceiling in kitchen; lower wall under three-compartment sink and floor around chest freezer are dirty.

Denny's, 6598 West Broad Street – The following violations were reported during a routine inspection: there is mold inside the ice machine; food debris found inside both microwaves on hot line; temperature of brisket portioned out in plastic bags is elevated; deep fryer baskets are in disrepair; cutting boards throughout kitchen are heavily scratched and damaged; fan guards in walk-in cooler are dusty; observed grease on ventilation hoods, equipment castors, hot line monitors and keypads, dials, drawers, skillet station, ANSUL system pipes and fryer cabinets; several areas are dirty (floors and walls throughout facility, fridge gaskets, sink handles throughout kitchen, bread storage trays, drying racks and door frames throughout kitchen); observed cracked tiles near ice machine and underneath dish table; there are patches of low grout in dish area; paint on walk-in freezer door is peeling; hole found in wall in dry storage area; dust is caked on lighting and wall panel above cook line.
